Patient Consent to share your full patient medical record

Unless you have specifically opted out, you will have a Summary Care Record (SCR) with core information about you.

Health services in Leicester, Leicestershire and Rutland have a system of sharing medical records between your GP practice and other NHS and social care organisations that care for you and your health.
The system allows the health and social care professionals who care for you to view information in your GP medical record. Viewing your record will help to improve the quality of your care and potentially save your life.
Unless you have specifically opted out, you will have a Summary Care Record (SCR) with core information about you: medications, allergies, and bad reactions. Your SCR can also be updated with additional information about your health after each contact with a heath and social care professional if you  consent. This may be of particular benefit to you if you have a long term health condition.
